Mammography Facility Survey

Your American Cancer Society is once again surveying all Indiana and Michigan mammography facilities and publishing the 2012 Mammography Guide for women in Indiana and Michigan. The results of the survey will be released in early October (Breast Cancer Awareness Month) for the purpose of promoting the importance of screening mammograms for the early detection of breast cancer and helping women identify and access their local screening facilities. 

Here’s what we’re asking you to do:

1. Fill out the 2012 Mammography Facility Survey. You may fill the survey out electronically and e-mail it back to lisa.dobias@cancer.org, or save the completed survey and fax it to ACS at 517-664-1425.
2. Please send completed surveys back by May 15, 2012.
3. If you are the contact for several facilities, please fill out a survey for each location.

In an effort to be more environmentally friendly, the Mammography Facility Guide will not be printed and distributed in hard copy. We encourage you to visit the website and review the guide as needed. You will be able to print the entire guide from the links below.

Rather than distributing the complete guide to the public, consumers will receive only the pages of the counties they request by calling 800-227-2345. The individual sheets will be accompanied by the definitions and technical notes and will be presented in a printed shell with more information about the Mammography Quality Standard Act (MQSA) and Medicare coverage of mammograms along with several toll-free resource telephone numbers. Patients may also review the guide online.
