• Make only one media entry per Entry Form.
• Entry forms and supporting materials must be submitted online or postmarked by Friday, July 6, 2012.
• All electronic files must be smaller than 5 MB. If you’re file size is larger, please download the file to a CD and mail to the American Cancer Society office.
