Thank you for taking the time to complete the Mammography Facility Survey for the purpose of publishing a consumer guide for women in Indiana and Michigan.
The guidebook is provided below as a reminder of your response from last year. We are now asking you to please review this information for accuracy and make any appropriate changes. A new survey form is also available below. Please review your information, complete and return the survey to us via fax at 517-664-1497 or e-mail to Teya at teya.trombley@cancer.org.
If there are no changes, please let us know that as well.
The mammography information will be provided to all media in Indiana and Michigan. In addition, a condensed version of the guide will continue to be made available to the public via the American Cancer Society local offices and cancer.org. Rather than distribute the complete guide to the public, consumers will receive only the pages of the counties they request. The individual sheets will be accompanied by the definitions and technical notes and will be presented in a printed shell with more information about the Mammography Quality Standard Act (MQSA) and Medicare coverage of mammograms along with several toll-free resource telephone numbers.
In the future as your facility goes through changes that would make the information in the guide obsolete, we ask that you let us know the changes. Please e-mail updates on your facility to Teya Trombley at the American Cancer Society at teya.trombley@cancer.org. We will change our database periodically and provide consumers with updated information.
