Request A Room

To request a room at the McConnell-Raab Hope Lodge, please have your doctor or social worker complete a Hope Lodge Referral Form and fax to the Lodge. The patient will be contacted to schedule a date and time to check into the Hope Lodge and to complete their mandatory tour and orientation of the Lodge.

It is advisable to initiate the referral process well in advance of the first date of treatment since patients are accepted on a first-come, first-served basis.
