This worksheet is a handy way to track all your medicines, vitamins, and supplements. It's especially useful to share with your doctor and your cancer care team. Take the completed form to all of your medical appointments, and keep a copy at home in case of emergency. If you take more than 8 medicines, print out 2 or more sheets so you can list them all. You may also need to print more copies as your medicines change. It is important to have an up-to-date list of all your medicines.
