Survivorship: During and After Treatment

ASCO Cancer Treatment and Survivorship Care Plans

It is helpful to keep records of your cancer treatments and the follow-up care you will need. The American Society of Clinical Oncology (ASCO) developed two forms that can help you keep track of these details. Keeping this information will help any health care providers you work with now or in the future better care for you.

These forms aren’t meant to provide a complete medical record, but they contain helpful information. Share them with other doctors and health professionals that you may see, such as your primary care provider.

ASCO Cancer Treatment Plan

The ASCO Cancer Treatment Plan is given to you before you begin treatment  and contains basic information about your cancer care history including:

  • The type and stage of your cancer
  • Treatment plan including medication doses, surgeries, and any other treatment that will be given
  • Potential side effects from treatment

The ASCO Cancer Treatment Plan is different than a Cancer Treatment Summary. A Cancer Treatment Summary is given to you after you complete treatment and is included as part of the ASCO Survivorship Care Plan below.  

ASCO Survivorship Care Plan

The ASCO Survivorship Care Plan contains details about what you will need for follow-up care after treatment including:

  • How often you should have follow-up appointments
  • Tests you will need
  • Possible long-term or late side effects of treatment
  • Options for healthy living 

In addition, the ASCO Survivorship Care Plan also contains the Cancer Treatment Summary which details:

  • The type and stage of your cancer
  • Treatment completed, including dates
  • Medical information about your time in treatment
  • Information on any ongoing treatment (such as hormone therapy), if pertinent
  • Ongoing side effects at end of treatment
  • Names and contact information for the doctors that treated you

Permission to use ASCO Treatment and Survivorship Care Plan Templates

Patients and other consumers may use ASCO’s Treatment and Survivorship Care Plan Templates without seeking permission from ASCO.

Practitioners may incorporate the Templates into their workflow and/or into their electronic medical records system without seeking permission from ASCO. If the templates will be altered in any way, including the addition of institutional branding, ASCO requires that all ASCO identifiers be removed.
